Synergies Method Coaching, LLC Agreement
This Agreement (the “Agreement”) is made as of [date] by and between
Synergies Method Coaching (the “Coach”), and _________ (the “Client”).
COACHING:
Description of Coaching: Coaching is partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. It facilitates the creation/development of personal, professional or business goals and develops and carries out a strategy/plan for achieving those goals.
COMMITMENT:
This Agreement is for an ongoing relationship between Coach and Client designed to facilitate the Client’s personal, professional and / or spiritual development.
Coaching is for individuals who are emotionally and psychologically healthy. Coaching is not advice, therapy or counseling.
1) Coach-Client Relationship:
- Coach agrees to maintain the ethics and standards of behavior established by the International Coach Federation (ICF) (https://coachfederation.org/ethics). We recommend the Client review the ICF Code of Ethics and the applicable standards of behavior.
- Client is solely responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results arising out of or resulting from the coaching relationship and his/her coaching calls and interactions with the Coach. As such, the Client agrees the Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any services provided by the Coach.
The Client understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.
- Client further acknowledges that he/she may terminate or discontinue the coaching relationship at any time.
- Client acknowledges that coaching is a comprehensive process that may involve different areas of his or her life, including work, finances, health, relationships, education and recreation. The Client agrees that deciding how to handle these issues, incorporate coaching principles into those areas and implementing choices is exclusively the Client’s responsibility.
- Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed. If the Client is under the care of a mental health professional, it is recommended they promptly inform them about the coaching relationship with the Coach. Should the Client already be under the care of a mental health professional, it is recommended that the Coach also be promptly informed.
- The Client understands that in order to enhance the coaching relationship, the Client agrees to communicate honestly, be open to feedback and help and to create the time and energy to take part fully in the program.
COACHING SESSION PROCEDURES:
Coaching sessions are conducted via telephone, Zoom, Google Meet, or through the Kajabi client portal unless otherwise arranged with the Coach. If the Client is over ten minutes late for a coaching session, the Coach will assume the session was canceled, and the Client forfeits the session.
The Client understands that in order to enhance the coaching relationship; the Client agrees to communicate honestly, be open to feedback and help and to create the time and energy to take part fully in the program.
Initial Session:—Coach will offer an initial free 30-minute consultation to determine whether Coach and Client would like to work together.
Commitment:—Client acknowledges that the best way to see results is to commit to the full Coaching practice and program. Should Coach and Client agree to work together following the Initial Session, Client agrees to purchase a program package with Coach. Each weekly session within the program is up to predetermined and scheduled at mutually agreed-upon times.
PACKAGE:

SERVICES:
Coach shall provide the Client with the following services on a WEEKLY SESSION basis (known as “Services”) to begin on a mutually agreed upon start date that will last for approximately 3 months, as stated in the package parameters.
- Number of Total Sessions: 9
- The Coach schedules Coaching Hours with a 3-week on, 1 week off session schedule during the approximate 3-month duration of the program. Coaching sessions are approximately 50-minutes. The non-coaching week is used to implement and reflect.
Coach will be available to Client by email and voicemail in between scheduled meetings as defined by the Coach as follows:
- Up to two (2) phone conversations of a maximum 15-minutes at no additional cost
- Additional phone conversations (maximum of 30 minutes) will incur additional $150 fees.
Coach shall provide Client with the following services:

● Custom Calendar Scheduling Link
● Session Prep Form
● Session Post Form
● Weekly Homework PDF’s or Workbooks
● Dedicated Support for ‘just-in-time questions’, homework review, & communication when needed
● Recording of Available Sessions
● Additional templates and documents to help make implementation quicker, faster, and easier
● My Proprietary C.O.A.C.H method that supports results
● Certificate of Completion
PAYMENT:
SESSION FEE: (1) payment of $2550 or (3) consecutive monthly payments of $875 (which includes a service set up fee), with the first payment due on the date of sign-up and subsequent payments due on the same day of the month for the following two months. Payment Plans are available at Coach’s discretion.
To ensure that all 9 sessions are used, please note that they must be completed within 6 months of purchase, with the first session scheduled and completed within the first 3 months. **Please be aware: you cannot defer monthly payments until you have used the first session.
Secured Payment (s) will be made through Clients ‘Client Portal’ within their Coaching Program, whereupon receipts from Stripe™ will be promptly provided. Monthly payment plans are available for financial convenience.
You must pay the full amount for your Coaching program, including any applicable sales taxes or fees. You are also required to provide a valid credit card for payment.
When you register and select the payment plan, we will charge your card the first payment and a set-up fee. We will then charge subsequent payments on the same day of each month for the duration of the program, totaling USD.
If you have selected a payment plan and miss a payment, we will change your account status to “delinquent.”
If delinquent, you will immediately lose access to future Coaching Sessions, your access to the Client Portal, and all Content will be revoked seven (7) days after your payment declined/or further payment arrangements have not been made in good faith.
If your account remains in delinquent status for longer than sixty (60) days, Coach reserves the right to report any delinquent balance owed to a credit reporting bureau and/or collections agency subject to the Coach’s sole discretion until the account is caught up and in good standing.
To be clear, Synergies Method Coaching, LLC is not a subscription payment model that can be canceled or a “pay in part” program where you can pay only for access to certain Sessions and not others. This is a full immersion program, and your payment plan is a convenience that we offer so that you can make the price sustainable.
LATE FEES:
If payment is not received by the Coach from the Client within fourteen calendar days of any payment date, the Client will be charged a late fee of 1.5% of the outstanding amount per week that payment is not received.
TERMINATION:
The Coach has the right to end the coaching relationship at any time.
Should the Client end or discontinue the coaching program, the Client agrees the Coach approves refunds on a case-by-case basis.
CANCELLATION POLICY:
Client agrees to notify the Coach a minimum of 24 hours in advance of the scheduled session. Providing Notice will not relieve the Client of any currently outstanding payment obligations. We do not credit or reschedule missed appointments (no shows). Coach reserves the right to bill the Client for missed sessions.
REFUNDS:
Because of the limited availability of coaching slots that cannot be easily rescheduled, and intellectual property of Coaching Content that is downloaded, if approved, approved refunds will be refunded at a flat rate of 20% (not to include any service fees) of what the Client has paid.
LATE FEES:
If payment is not received by the Coach from the Client within fourteen calendar days of any payment date, the Client will be charged a late fee of 1.5% of the outstanding amount per week that payment is not received.
CONFIDENTIALITY:
This coaching relationship, as well as all information (documented or verbal) that the Client shares with the Coach as part of this relationship, is bound by the principles of confidentiality set forth in the ICF Code of Ethics. However, please be aware that the Coach-Client relationship is not considered a legally confidential relationship (like the medical and legal professions) and thus communications are not subject to protecting any legally recognized privilege. The Coach agrees not to disclose any information pertaining to the Client without the Client’s written consent. The Coach will not disclose the Client’s name as a reference without the Client’s consent.
Confidential Information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Client; (b) known to the public or in the Client’s industry; (c) obtained by the Coach from a third party, without breach of any obligation to the Client; the Coach independently developed (d) without use of or reference to the Client’s confidential information; or (e) the Coach is required by statute, lawfully issued subpoena, or by court order to disclose; (f) disclosed to the Coach, and because of such disclosure, the Coach reasonably believes there to be an imminent risk of danger or harm to the Client or others; and (g) involves illegal activity.
The Client also acknowledges his or her continuing obligation to raise any confidentiality concerns with the Coach in a timely manner.
Release of Information The Coach engages in training and continuing education pursuing and/or maintaining ICF (International Coach Federation) Credentials. That process requires the names and contact information of all Clients for possible verification by ICF. By signing this agreement, you agree to have only your name, contact information and start and end dates of coaching shared with ICF staff members and/or other parties involved in this process for the sole and necessary purpose of verifying the coaching relationship, no personal notes will be shared.

According to the ethics of our profession, topics may be anonymously and hypothetically shared with other coaching professionals for training, supervision, mentoring, evaluation, and for coach professional development and/or consultation purposes.
LIMITED LIABILITY:
Except as expressly provided in this Agreement, the Coach makes no guarantees, representations or warranties of any kind or nature, expressed or implied regarding the coaching services negotiated, agreed upon and rendered. In no event shall the Coach be liable to the Client for any indirect, consequential or special damages. Notwithstanding any damages that the Client may incur, the Coach’s entire liability under this Agreement, and the Client’s exclusive remedy, shall be limited to the amount actually paid by the Client to the Coach under this Agreement for all coaching services rendered through and including the termination date.
ENTIRE AGREEMENT
This document reflects the entire agreement between the Coach and the Client, and reflects a complete understanding of the parties regarding the subject matter. This Agreement supersedes all prior written and oral representations. The Agreement may not be amended, altered or supplemented except in writing signed by both the Coach and the Client.
DISCLAIMER:
The role of the Coach is to facilitate the Client towards his/her own choices of action. Whether to make a particular choice or take a particular action is entirely the responsibility of the Client.

Synergies Method Coaching Group, LLC, offers resources for goal-setting, positive habits, spirituality, and becoming your best self. You need to know that your ultimate success or failure will result from your own efforts, your particular situation, and innumerable other circumstances beyond my knowledge and control.
Client waives and releases Coach from all actions, claims, or demands that Client, its assignees, heirs, guardians, and legal representatives now have or may have for injury, illness, death or damage to property resulting from or relating to her participation in coaching sessions contemplated by this Agreement.
